Appointments must be re-scheduled/cancelled within a 24 hour period and by voicemail or on-line. The fee for the missed appointment or any appt. not within the 24 hour period is the cost of the session due within 7 days. Fee may be waived due to family emergency, or acute illness.
If you miss an appointment or do not re-schedule/cancel within the 24 hour notification period, with the Family/Friends Wellness Package/Pregnancy Package, or any other plans/packages, barring a family emergency or personal crisis, the cost is the Massage Certificate or gift certificate. Reasonable effort will be made on the part of Siete Lobos Healing Arts to ensure that you will get the full benefit of the plan/package/gift certificate.
The therapist retains the right to refuse service or to discontinue a session due to inappropriate behavior, language, not showing up for a scheduled appointment, if the client is under the influence of alcohol or an illegal drug, chronic and habitual tardiness, if the client has a contagious condition (harmful to both the client and the therapist!), owed re-scheduling/cancellation/no-show fees, or for anything outside the boundaries of Therapeutic Massage and Treatment. If the therapist, Lisa Eileen Lentz, LMP, & Michael Richard Lentz LMP< Siete Lobos Healing Arts ends the session early due to any of the reasons listed, it is at her discretion and judgement that a refund will or will not be issued to the client. Clients will please inform Siete Lobos Healing Arts of any new medical conditions, medications, or limitations to Therapeutic Treatment, including, but not limited to, new diagnoses by your Primary Care Physician, Chiropractor, or any other qualified health care provider. If it is appropriate, the client will call ahead and give Siete Lobos Healing Arts an opportunity to call and check in with their doctor or qualified health care provider to ensure the appropriate measures are taken and the appropriate modalities utilized. The client recongnizes that verbal or written consent needs to be given to the qualified health care provider prior to Siete Lobos Healing Arts contacting them. The client also needs to give verbal or written consent for Siete Lobos Healing Arts to share confidential treatment information to any person other than themselves. Out-of-State & International clients please read!: Please note the means of payment in the Out-of-state clients section in the menu. By Booking with Lisa Eileen Lentz, LMP, & Michael Richard Lentz LMPm Siete Lobos Healing Arts, the client does agree to each and every term and condition listed above and will abide by them. Paypal Invoices: Paypal invoices are DUE within 48 hours of being emailed to you. If the paypal invoice has not been paid within the 48 period, a new invoice will be issued that incorporates the cost of the Paypal service fee (normally paid by Siete Lobos Healing Arts) and is due with 48 hours of being emailed to you. Siete Lobos Healing Arts Draping Policies: We offer Therapeutic ONLY services. We have a sheet only draping policy, no exceptions will be made. |
