PIP Claims/L & I; HCFA Reimbursement


PIP/L& I Claims are on a case-by-case basis and must be verified with the Insurance company and Claims Adjustor prior to the appointment.  Copies of all pertinent documentation, and a prescription from the doctor or attending physician (d.c, Lac, etc...), must be presented at the first session.   The cost of treatment work is $100/hour due to the paperwork, delay in payment, and faxing fees.   If you are already working  with another massage practitioner, please let  me know, as it  can alter the reimbursement status with the Insurance Company.   All  PIP clients are responsible for missed or less than 24  cancellation Policies, which is the cost of the PIP session booked.     
 
I will generate an Insurance form for you to present to your insurance company if you are seeing me for Blue Cross, King County medical, or any other Non-motor vehicle accident issue.   Reasonable effort will be made to ensure that you are reimbursed for the maximum amount owed you.  Please check with your insurance company as to the specifics of your individual or group policy. 
Copying & Faxing fees may apply for all non-Pip services.  In the event that charges apply, you will be sent the details via email or phone message.
Records, when requested, will be sent within the 15 working day limit, as set forth by the laws governing the release of records.
