PIP Claims:
(You will find PIP claims on the SCHEDULE at the top)
I am now accepting new PIP clients. Please bring the following information in with to your first appointment:
1. A prescription from your doctor for the therapeutic massage. (Including diagnostic codes)
2. The mailing address, claim number, claim manager's name and number, and authorization code.
3. Any other documentation that might be helpful to me. (including the name of your attorney so that I may send them chart notes should they need them)
I will mail your claims in for you. I will gladly make a copy of the claim form for you upon written request. It will be mailed to you the same day that I mail out the claim to the insurance company.
I kindly ask for 24 hour notification should you need to cancel or re-schedule an appointment. Family or personal emegencies will be taken into consideration. Should you not show up for your appointment of fail to contact me within the 24 hour period, the fee is the cost of the treatment, which is $100 for an hour.
This is due no later than 7 days from the date of the original appointment.
It is important to keep your appointments for the healing process to progress and for good faith effort on your part for any legal issues with the accident.
